For Family and Community Services (FCS) Employees:
FCS Permanent Employees are granted default VPN accounts upon hire.
Non-permanent employees, contractors and sponsored staff require administrative and security approval prior to VPN account set up.

Instructions for Completing and Submitting the VPN Form:Please complete the OIT VPN request form and obtain your supervisor and director’s signatures. When signed, scan the form to a PDF and email to the ITS HelpDesk at
